GENERAL EXPLANATIONS
Personal data owners, defined as the data subject in the Personal Data Protection Law No. 6698 (KVKK), have been granted certain rights regarding the processing of their personal data in Article 11 of the KVKK. Pursuant to paragraph 1 of Article 13 of the KVKK; the applications to be made to our Institution, which is the data controller, regarding these rights;

Your applications submitted to us will be answered as soon as possible and within thirty days at the latest, from the date of receipt of your request, in accordance with the second paragraph of Article 13 of the KVKK, depending on the nature of the request. Our answers will be delivered to you in writing or electronically in accordance with the provisions of Article 13 of the relevant KVKK.

This application form has been prepared in order to determine your relationship with our Institution and to respond to your relevant application in a correct and legal time, if any, of your personal data processed by our Institution. Our Institution reserves the right to request additional documents (identity, driver's license, etc.) for identification and authorization determination, in order to eliminate legal risks that may arise from illegal and unfair data sharing and especially to ensure the security of your personal data. In the event that the information regarding your requests submitted within the scope of the form is not correct and up-to-date or an unauthorized application is made, our Institution does not accept any liability for such false information or requests arising from unauthorized applications.

If you are applying on behalf of someone else, you must submit documents that show that you are authorized to apply and that are issued/approved by the competent authority (document showing that you are a parent/guardian, power of attorney with special authorization, etc.) along with the application form. Likewise, in order for our company's response to the application to be hand-delivered on behalf of someone else, the said documents must be submitted.
